To understand how a coma occurs, one must first have a general idea of how consciousness is maintained in a normal brain. We know that in the lower part of the brain, called the brain stem, there is a cluster of nerve cells responsible for maintaining the state of wakefulness. These nerve cells use excitatory neurotransmitters such as acetylcholine, norepinephrine, dopamine and glutamate to excite groups of cells in the upper part of the brain called the forebrain.
